Service Request Form

Service Rate: There will be a dispatch fee $90.00 plus the hourly rate of $180.00 per hour which will be rounded up to the nearest fifteen-minute interval. There is a minimum charge of $135.00. All calls run between the hours of 4:00 p.m. and 7:00 a.m., as well as calls run during weekend hours, will be subject to a rate of 1.5 times these regular stated rates. Flat-rate pricing applies to certain services, which will be disclosed before work begins.

Material: Material charges will be added to invoices accordingly.

Payment: This account will be C.O.D. In the event your issue takes more than one day to resolve, payment will be collected at the end of each day. We accept MasterCard, Visa, Discover, American Express, and cash. Any delay caused by events beyond the control of Eddie’s Plumbing Company shall not constitute abandonment and shall not be included in calculating time frames for payment or performance.

Permits/inspections/zoning: Eddie’s Plumbing Company shall comply with all local requirements for building permits, inspections, and zoning.
