Service Request Form

Service Rate: There will be a dispatch fee $90.00 plus the hourly rate of $180.00 per hour which will be rounded up to the nearest fifteen-minute interval. There is a minimum charge of $135.00. All calls run between the hours of 4:00 p.m. and 7:00 a.m., as well as calls run during weekend hours, will be subject to a rate of 1.5 times these regular stated rates. Flat-rate pricing applies to certain services, which will be disclosed before work begins.

Material: Material charges will be added to invoices accordingly.

Payment Terms & Conditions: Payment is due upon completion of service. For multi-day jobs, payment for each day’s work is due at the end of that working day. We accept cash, checks, Visa, MasterCard, Discover, and American Express.

Returned checks will be subject to a $50.00 processing fee plus any applicable bank fees, billed on a separate invoice. Certified funds may be required for all future payments following a returned check.

Invoices not paid within 30 days will accrue a late fee of 1.5% per month on the unpaid balance until paid in full.

Delays caused by circumstances beyond Eddie’s Plumbing Company’s control do not constitute abandonment and are excluded from payment and performance timelines.

Permits/inspections/zoning: Eddie’s Plumbing Company shall comply with all local requirements for building permits, inspections, and zoning.
